HOW TO COLLECT: New and existing customers of Hooper-Holmes Canada Limited are eligible to collect    
AIR MILES^® reward miles.

• Collect 3 base AIR MILES^® reward miles with every online case entered and completed via our Hooper-Holmes Canada Limited website.
• AIR MILES^® Collector number must be entered into the Hooper-Holmes Canada Limited website in order to begin accumulating reward miles. Cases entered prior to the addition of your Collector’s number will not be eligible for reward miles.
• AIR MILES^® will be granted on a monthly basis – please allow up to 6 weeks for processing.
• Hooper-Holmes Canada Limited assumes no responsibility for the maintenance, verification or accuracy of             AIR MILES^® information entered into our website.
• If the customer is not the AIR MILES^® Collector, he/she must notify the AIR MILES^® card holder of any changes,        reward miles granted, etc.
• AIR MILES^® reward miles are not convertible to cash.
